In the next couple of days, employees of UP who worked full-time during any month in calendar year 2015 will be receiving a form called Form 1095-C via US mail. This is an IRS form that UP is required to send to employees starting this year. Employees who did not work full-time during any month in 2015 will not receive this form (for example, most adjunct faculty members).
